P-062 SEVERE COMPLICATION AFTER EVENTROPLASTY: NECROSIS OF RECTUS ABDOMINIS MUSCLES

Abstract Aim Rectus abdominis muscles are supplied mainly by superior and inferior epigastric arteries, forming an arcade between them. The ligation of one them during a surgical intervention usually has no clinical repercussions. However, when both arteries flow interrupted, there is another scenario. Material Methods We describe the case 74 years-old woman who was admitted for elective surgery fourth recurrence midline eventration (M2-5W3 European Hernia Society Classification). treated using preoperative botulinum toxin Rives-Stoppa technique. On second postoperative day, patient suffered cardiorespiratory arrest due to pulmonary embolism, therefore anticoagulation necessary. After showed severe anemia (Hemoglobin 5 g/dL) tomography computed scan images revealed big hematoma abdominal wall with active bleeding. Results Embolization epigastric, right mammary left pectoral performed interventional radiologists. 12 days, cutaneous wound dehiscence serohematic foul-smelling fluid. detected necrosis rectus we debrided wound. It daily cures vacuum therapy without mesh removal, achieving progressive closure. six months follow-up, signs recurrence. Conclusions Ischemia complication after eventroplasty that can be debridament necrotic muscle therapy, removal if it sufficiently extended along healthy cover defect.
